    Hi

    We are new to Jotform and need to get a form built quite urgently for our website, we were just getting started and then the site crashed and now our account is suspended due to 'suspected phishing'. We imported our existing application form from another system, and were in the process of reviewing it and editing it in Jotform. We are a university and recruiting students, this is our application form.

    Please can you get the account re-activated asap and how do we prevent this happening again?

    Hi there,

    Your account has been suspended due to asking for login credentials on your form as shown below.

    Also, your account has been suspended due to phishing activity within your form in which you ask users for "ID information" as shown below:

    Furthermore, online Form Builders, including Jotform, are sometimes being used for identity theft and we go to great lengths to prevent this. That's why collecting these types of data is considered as phishing activity, as stated in our Terms of Use. If your purpose is not to collect these types of data from general public, but from your own customers, then you need to comply with two things:

    1. Add the Terms & Conditions Widget (or the Short Scrollable Terms Widget) for your users to read and agree on.

    2. Add a Signature Widget so your users can sign and agree to provide such sensitive information.

    However, if you don't want to add these widgets to your form, you can use encryption instead.

    We are patiently waiting for your response with which you explicitly confirm that you agree on removing the offending fields shown above, besides applying the required changes, so that we can act further and reactivate your account.

    Dear Sir/Madam,
    Thank you for getting back to me, I can confirm that we won't be using the login fields we do not need them and do not intend to every use login details in this, or any other form. These fields pulled across from an existing form that is hosted on another platform where this is necessary, we used the very handy tool to copy this form and were in the process of deleting these elements in our new Jotform form but then we were suspended about 30 seconds after the form was copied across so couldn't continue to edit the form.
    Is there any way to use the 'copy form' feature and remove the offending fields before we are kicked out? Or do we need to start from scratch to avoid this completely? This is a very long form so we were hoping to avoid this. The reason we are moving to Jotform is the save and return feature and this will replace the legacy 'login' fields from our old form with another provider.
    We will add the T's and C's, and signature widget of course. It would be our intention to do this, we are aware we handle lots of personal information and the terms will be available to everyone in our TCs and Cs that they will need to agree to prior to submitting any forms to us.
    Please let us know if there is anything else we need to do to get started with our form creation as soon as possible.

    I've successfully removed the offending fields and reactivated your account. You may log in now with no issues at all.

    Please follow the instruction provided in my first response if you intend to re-add the fields.
