we have a questionare on our website that is used to collect registration information from Drs requiring electronic delivery of reports.
The last field in the form is the one that contains the Drs Name, Speciality and Professional Registraion number. Recently this field has been visable via Jotform, but not in the reply email that was sent.
Previously this worked without issue.
Today I deleted that particular field, but now when the form is completed, the field is duplicated 8 times. I've checked the properties of the field and created the field individually instead of just copying each time.
Any help would be greatly appricated.

Your form does contain the eight (8) fields mentioned to the form. Please check screenshot:
Therefore, you should receive it to your email on the same number. The entries may be different per user submissions. I've check the form in the form builder and can also see the fields there. Please kindly check it again and make sure to save the form for the changes to take effect.
I would also suggest cloning the form and do the changes to the clone version and use the clone it instead of the original one. Of course, you should change the form codes on your website. I would also suggest embedding your form using our iFrame Method: Getting the Form iFrame Code.

I'm sorry, I just checked your form notification email settings and could see that the field tags are the same for the eight fields, causing the duplications. I've fixed it for you. Can you check it again, please? If there still an issue, I would suggest to re-create the settings. First thing, delete: How to Delete Notifications/Autoresponder. Second, re-create: Setting Up Email Notifications.
