I have followed the instructions and my submissions are created as a ticket in Zendesk however the complete forms is not in the ticket, how do I get the completed form/submission to show in the description of the ticket?

Greetings.
Is submission data from your form not available in Zendesk? Is the problem only with Ticket Description? Do you receive Email, Name, Status, and Type? Could you please send us a screenshot of the ticket you have received? You can click here to see how to share images with us.

I would recommend uploading it to a cloud drive (e.g. Google Drive or DropBox) and share the link here so we can view it.

Also, I've just integrated my test form with ZenDesk and made a test submission. However, I cannot replicate the issue.
The created ticket has data in it:

Okay. So as far as I understand, you would like all of the data received via email to be added as well in the ticket description in Zendesk.
If so, you can manually add all the fields into the ticket description field of the integration:
And here's how it shows in Zendesk:
