How can I stop or decline form submittal when products is out of stock

I want to design a form for doctors that are distributing the COVID-19 vaccine. I need to limit the amount of forms that they receive based on the available doses.

Which template should I use and how to I add a form decline due vaccine unavailability?

Is it via booking? Meaning, you accept vaccine appointments on a limited basis per day. If that's so, you can use the Appointment field. Here's a guide on how-to-schedule-an-appointment.
