Issue with tab display of form

I used a template to create my own Church directory form. When I click the link, all of my information and my form load, but the browser tab shows a different church's (Harpeth Presbyterian) name. I tried to modify the specific URL for my form, and this does not change.

Here's the link to my form:

    https://form.jotform.com/jory.harlan/st-bernards-church

I'm assuming that Harpeth Presbyterian Church is the originating source of the template I used. Is it possible to make the browser tab display information specific to the actual form? I noticed it right away, so I'm sure it could be confusing to my Parish members who are clicking the link to complete the form.

Any help you are able to provide is much appreciated. I'd like to get this form published to my Parishioners as soon as possible. Thank you!

    Hello,

Thank you for reaching out to us. To change the title name that appears on the browser tab, please click "SETTINGS" at the top of your screen and make a change to "Page Title" under "FORM SETTINGS" tab.
