Your SMTP credentials might not be updated. You can try to update your SMTP credentials by following this guide https://www.jotform.com/help/241-how-to-edit-delete-sender-emails-on-smtp-settings or just use the noreply@jotform.com as your sender email.

Dear galenoralsurgery,
There was an error on your SMTP configuration. Due to that error, our system
automatically changed your email sender's address for your form ID

account.
Please log in to your JotForm account (http://www.jotform.com) to correct your

However i do not see where i need to make the change to correct the problem as i receive all my other forms. There appears to be a change in the appearance in your dashboard from when i originally set this up. I needed in the past to put in the smtp settings as otherwise i was not predictably receiving the submission. currently i can only get this form “ patient registration” via log in to jot form it is not being emailed to me. the link below describes a general email for the account not the individual form. Please help me correct this problem as i am not receiving the most important form “ patient registration”

Thanks for the response. Can you please confirm that you are still not receiving an email even if you make a test submission right now? I just checked your form and it seems to have switched back to noreply@jotform.com so please make a test submission and let us know if you receive any notification via noreply@jotform.com sender address.
Looking forward for your response.
