This form https://form.jotform.com/210814259392053 was found on your account (ajmoren1) as suspicious of phishing activity because you're collecting credit card details in plain form fields.

Due to our PCI certification, we are required to deny the collection of credit card details on regular forms.

However, we have other PCI-compliant ways to do this. All you need to do is choose any of the payment processors available on our platform in order to integrate with your forms. You still won't be able to collect credit card details directly though.

If you only want to collect your customer's payment details and charge them at a later date, you can take advantage of a feature called "Payment Authorization". To learn more, kindly follow this guide on How to Enable Payment Authorization.

We’ve removed the credit card fields on that form, however, aside from that, the forms in your account also have this copyrighted logo of a private company.

We understand this might not be the case with your form, yet we had to act according to the general rule to be in compliance with our Terms: Free accounts with non-corporate emails showing suspicious forms must be suspended immediately.

In order to reactivate your account, we will have to delete the offending forms.

Or, kindly provide us the following information:

    1. The URL where your form is used.

    2. Your website or proof of affiliation to a legitimate institution.

We also need you to change your registered email to a corporate one to avoid getting flagged again by our phishing detector.

Send these proofs to reviewers@jotform.com with Thread #2994215 as your subject. We will let you know about the status of your account after a thorough review of your proofs.
