why did i need to sign up for this it has nothing to do with insurance

Please note that JotForm is an online form-building service that has many users, some of which work in insurance companies. It is possible that your insurance carrier is using JotForm and they have sent you a form to fill out.

Unfortunately, since we do not know who the form belongs to we are not able to assist you further. However, if you can share the form URL that was shared with you, we will check to see why you are being asked to create an account.

Do note that it is not necessary to create a JotForm account to sign up for insurance. However, what is most likely happening is that the person who created this form set it to private and it requires you to create an account in order to fill out the form.

You may want to reach out to your insurance carrier directly and ask them about their forms and let them know that you are having trouble filling it out.
