I regret to inform you that the possibility of restoring your form is highly unlikely. A few other users have had the same problem and their forms were irretrievable.
If you can remember what fields the form contained and how they were positioned, replace them. Once replaced, clone the form so that you would have a backup should a similar situation reccur. Also for backup purposes, periodically download submissions and user uploaded files to your computer.
Just in case there is any possibility of restoring the form, a ticket will be submitted for you.

I apologize but it looks like you were affected by a server problem that happened almost three weeks ago. As per our higher ups, there isn't a convenient way of restoring forms that were affected by that unfortunate incident.
Our advise to you is to recreate your forms. We are sincerely sorry about that.
