I have several conditions on my form and they worked fine prior to today. Today, it seems only the first condition works and the remainder of the conditions do not work.

I think that there is a conditions conflict issue. Let me please explain this.
For example, your first condition:
This condition also means:
If Truck Classification field Is Filled
If Capacity field Is Filled
Then Show - Can you provide the total...
Then Show - Enter the total number of...
Then Show - Total Number of Service ...
Then Show - Enter total number of se ...
So this condition will override any other conditions on form.
As a solution, you can use only Show rules within your conditions.
For example, delete your first condition with Hide rules, and then use Show rules only:
Also, you are missing a Submit button on your form, so it is not possible to submit the form at this moment.
Note, you can clone the form and practice on the cloned version.

Understood. I only added the First condition (with all the Hide conditions) because all of a sudden all of the fields where showing and I had made no changes to the form. Upon further investigation on the forum, I saw that there where issues with conditions yesterday (site wide) and this could have been the possible cause of the form not working properly yesterday.
At this time, I do not need a submit button. The form is simply for calculations at this time.

I can confirm this, yesterday there was a bad commit that caused some conditions issues. As far as I know, it was fixed by our developers. We are very sorry for the inconveniences caused.
