There is no PDF provided of the original form I filled out

    Hi. I hit a dead end when directed to submit my own form after signing up for the COVID vaccine. There is no PDF provided of the original form I filled out and copying the URL of the one that was sent in the JOTFORM welcome email does not work. Is every person wanting to sign up to get on the vaccine waiting list expected to navigate this esoteric system?

    If you didn't receive a copy of your submission in PDF while filling a form to get a vaccine, you should get in touch with the people who created said form so that they can assist you with it.
