No Signatures are coming over on Good Drive PDF's. I've tried everything.

I have cloned your form and see that the Signature widget is in the form. However, the PDF file had failed to upload. Could you kindly re-upload your PDF file into your form again?
In addition, kindly try to remove and integrate with Google Sheet too.
