    Goodmorning,

    My name is Imani Gerald and I am the front desk administrator at Carolina Women's Physical Therapy and Wellness. We have been utilizing JotForm as a part of our new patient intake process and it has been successful; however, there was one glitch that occurred today. A patient submitted her form through the link and when I logged into JotForm to print the papers, the submission was blank; however, I received an email saying that her forms were completed with the information. In the email, I could view all of the information submitted; however, when I went to the actual website her form was blank. I was hoping that someone from the support team can get back to me as soon as possible. Thank you!

    Thank you for reaching out to us. I've checked your form named "CWPTW Patient Intake Form" and it seems all submissions are displayed correctly. May I ask if you manually edited the submissions or there was a temporary delay in submission displayed?


    If you are still having issues viewing the submissions, please try to clear the form cache to see if that would fix the issue.
