How can I have columns in a table that are connected to different forms?

I'm trying to set up a table that tracks data across three forms - I'm stuck on how to create three look up columns that pull data from three different forms.

Can you give me some instructions on what is a connection column and how to set it up - and how to pull data from three forms into one table?
