Well, I was able to embed the form using a widget. However, There are a few problems. I have viewed the form on Firefox, Chrome, and Safari and can't fill out the form with any of those browsers. The interactive function of the form seems to have disappeared. Also, I can't get rid of the word "Click here to edit" because when I delete those words the form is also deleted.

When I view the form on Internet Explorer 11 this is what I get:

"This content cannot be displayed in a frame

This content cannot be displayed in a frame. To help protect the security of information you enter into this website, the publisher of this content does not allow it to be displayed in a frame."

You can view my form here:

The form I imported is here:

I was able to view the PDF Form using the browsers mentioned. I'm currently using Windows 8.1. What's your OS?

I've fixed the alignments and the click to edit text. The PDF file is part of the first PDF Embedder widget. There's a duplicate one so I removed that.

Let's go back to the PDF File behavior. I know that your PDF is fillable but take note that any information filled to that PDF file will not be captured when someone hit submit button to your form. The PDF Embedder widget would only consider the file as "Read-Only.

By the way, I forgot to mention that you can have the fillable PDF using the Free Text HTML field per the guide you commented on: How to Display a PDF Document On Your Form.

    Add the codes as:

    <iframe src="//quilt-festival.com/entry_form.pdf" style="width:100%;height:500px;border:none;" frameborder="0"></iframe>
