Card Layout Dynamic Floating Comments Field

    Our client really likes the card layout; however, they need one of the two following options:

    1) A floating comments field that persists on each question to allow the user to enter additional feedback.

    2)

    The flexibility of adding multiple fields on a single card. What they want is a checkbox that toggles a hidden textbox on the same card. I don't see any way of including customizable multiple fields other than the multi-line question widget, which doesn't accomplish what we need.

    I'm aware that we could use the classic view to accomplish what we want with css, but the requirement is to use the card layout. Is there a way to custom-create a widget that would accomplish what we need?
