I tested the form with one of the emails on the domain pvcdme.com the mail logs indicates
that the notification was sent:
Jun 14 23:19:19 yang sm-mta: p5F3JIvQ031213: to=, delay=00:00:01, xdelay=00:00:00, mailer=esmtp, pri=126388, relay=sm.primenetmail.com. [18.104.22.168], dsn=2.0.0, stat=Sent (Message accepted for delivery)
Have you looked into the bulk or spam folders? The mails might have
been caught in your email's spam filters.
