Why am I not receiving emails.

    Email alerts are set, I don't receive emails to any @PVCDME.com  ... here's form URL http://www.jotform.com/?formID=11405606332

    I tested the form with one of the emails on the domain pvcdme.com the mail logs indicates
    that the notification was sent:

    yang sm-mta[31215]: p5F3JIvQ031213: to=, delay=00:00:01, xdelay=00:00:00, mailer=esmtp, pri=126388, relay=sm.primenetmail.com. [], dsn=2.0.0, stat=Sent (Message accepted for delivery)

    Have you looked into the bulk or spam folders? The mails might have
    been caught in your email's spam filters.
