I have a patient form that I need clients to fill out. my first question is can I create form that looks exactly like the attached png. & how can I receive results that look (is formatted ) exactly like the attached png. I would just like to print out the results page that looks exactly like the form they are filling out.

It's difficult to see your image, but you should be able to get your form looking very close by using your own CSS. To do this
1. Open your form in JotForm
2. Click on 'Setup & Embed'
3. Choose 'Preferences'
4. Select 'Form Styles' and in the Inject Custom CSS. See the following example.
Note that you will be changing the look of the form considerably, so you'll be adding allot of CSS to get this working. I would recommend hiring a developer if you really need it to look exactly the same.
I'm afraid that the results page wont be able to look like this. The best you could do is to export the results as an excel sheet and then merge that into some Excel template.
