    Hello Rachelle,

Sorry about that, and thank you for letting us know.

We do not offer phone support, but our forum is monitored constantly by a team of experts in providing you speedy and effective responses.

I cloned your form from the URL you provided and found that the condition rules have some errors, apparently there is a rule missing:

We'd like to help you resove this.

Please walk us through what you would like to have the form do in the following situations according to your question (or clarify it in your own words by responding to this thread):

    1. When would you like the concierge section to appear?

    2. When would you like the First Name and Last Name to appear?

In order to get quicker and accurate responses for each issue, Please open another thread here, clarifying which date field is not appearing on email notifications.

    Thank you for your patience.

Hi, together with my colleague. I'm also here to help yah. When I checked your form, there was already a condition set. But can you please be more clear about the problem? What you really expect to the form? what form fields to be hidden and shown when you tackle other inputs?

We expect more informations from you, this will easily help us to the problem.

    Thanks,
    Kenneth
