Hi I need to add a Drop down before names so we can identify: Mr., Mrs., Ms. of each name in the form. I was not able to find out how to do it. Would you please let me know if it's possible, and if so, how can I do it?

While editing your form:
Under Quick Tools add Full Name form element to your form.
Click on the Full Name form element and on the top toolbar click on "Prefix" button.

Is there anyway I can make the prefix a dropdown that users can click on it and select one option in stead of typing in? That's what I couldn't figure out.

There is currently no way to replace the built-in prefix sub-field in the Full Name quick tool. If you don't mind, you can create a separate dropdown for the prefix then shrink it to align with the Full Name quick tool. The downside to this is that the prefix value will be separated from the full name value in the notification mails and when viewing the submission entries.

To shrink the dropown field, right-click on it then click 'Shrink'. However, making the dropdown field appear as if it's a part of the full name field would entail a lot of steps, including injecting some custom css.
I went ahead and created a modified clone of your form inside your account. The name of the clone is [Jotform Support] Call Campaign 2011 to Renegage Shoppers. Feel free to modify it and make it your own.
