I tried deleting one set of conditions and now everything is gone! Is there ANY way to fix this or access and older version of the form. THere were so many conditions set up, i have no idea how to re-do them all.

I realize I had a older version of the form, but my conditions aren't going to the correct page. A very helpful technician fixed it for me (which i then deleted accidently). So if I can't get my conditions back on the form above, can someone help me fix the conditions on this form: http://form.jotformpro.com/form/33454321093953
We have issues with: Choose (one) primary focus of your MLK Day Project(s) and Do you have another focus for your MLK Day Project(s)? where they are not going to the correct focus page.

Sorry, back to the original problem. the cloned form i spoke of in my secound email was a different form. Please disregard my 2nd email.

Hi when I checked your form "MLK Day 2014 Grantee Performance Measures Tool", I can confirm that there is no conditions set which I presumed it was really deleted. We will try our best to recover the data you have lost but there is no guarantee that we can get it back completely.
But for now, can you tell us what exactly you did why this conditions are gone from your form?
Because if you really delete all the conditions, you should have been warned by the wizard for something like this
- when you clicked the OK button, then all your conditions will be gone. can you confirm?

I did receive that message, but i thought it meant all the conditions from the form. If there is any way to save it I would appreciate it. thank you!!

Yes, that message is a warning that 'ALL' conditions on the form will be remove.
Let see what we can do about that, because me myself I'm not really sure if its possible to restore.
But I already forwarded this to our higher teams, let see what they can do about it. Just please just be more patience. We will update this thread once we got something.
