We have form V8 issue recently but had been fixed already.
You just need to refresh your browser and the form will be viewable properly.

Yes, that's a good point. It has occurred to me as well but unfortunately, that's the only remedy for the issue.

Since the form is embedded in a web page, perhaps it would be a good idea to put a notice on the same page telling users that if they encounter the issue they should refresh their browser.
