Can you create a form that once submitted can post results of the form?

Asked by bbeatty43 on December 17, 2013 at 09:34 AM

    For example.

I conduct security assessments on different properties.  So I use the form as a way for clients to take an assessment of their property to find out what their security risk levels are.  Each question that is answered yes equals 1 point.  The points are totaled up at the end of the form and the client then falls into a risk category.  Is there anyway to do this.  Thanks.

Seems like you are looking for a way to measure data, by assigning points based on the type of answer, then assigning a category.  This type of form analytic is not currently measured.

But if you explain in detailed the scenario and the desired results , then we could see if this can be done with a work around or recommend to you a 3rd party solution

I believe I kinda get what you are trying to do. You basically want to use your " Threat Level " as a response measuring system for the form based on a certain amount of points.

Did you have any calculations to put on your form? Or was the " 1 Point " just a general statement?

I think your best bet would probably be to create a " SUM " field by using a textbox on your form. Then you could use our Calculate Field ability to gather assigned points from each of these fields depending on how they are filled in but be advised that this will only work with certain fields and the radio button is not one of them. This would allow you to then use the created SUM field as a numerical basis to create conditions which will split up and show one of the 4 Threat Levels based on the assigned range.

Thanks for the information. I will give it a try. What type of field will
work since the radio buttons are out? Thanks in advance.

    Hi,

You can do it this way, example form: http://form.jotformpro.com/form/33501528710951?. The 1 and 0 prepended on the text options are used for calculation.

You have to add a total textbox field to calculate all the selected options. Check the following image for example:

    Hope this helps. Thanks!

    Hello,
I've tried to get the form to show results like yours but I seem to be
having some issues. Here is what it gives me to put in the calculated
    fields:
    {q1Lights}+{q2Sprinkler}+{q3Alarms}+{q4Are}+{q5Trash}+{q6Pool}+{q7Have}+{q8H
    as}+{q9Entry}+{q10If}+{q11Environment}+{q12Courtyard}+{q13Loitering}+{q14Gra
    ffiti}+{q15If}+{q16Vandalism}+{q17Loud}+{q18Parking}+{q19Piggy}+{q20Theft}+{
    q21Has}+{q22Vehicle}+{q23Burlaries}+{q24Police}+{q25Cameras}+{q26If}+{q27Doe
    s}]
Then the total field looks like this when filled out:
    1+1+1+1+0+1+1+1+1+0+1
What do you think might of gone wrong? And how can I fix it? Thanks.

That is because you just added 1 and 0s to each radio buttons. If you want to assign different or higher values, then you can do it such as 3-YES, 5-No.  And the calculated field should make the correct math

Anyhow, can you please provide us with the form link and explain to us the desired escenario?
