    Hi Max,

    Is this the form in question: http://www.jotformeu.com/form/32037057946356 ? The form title is fine. The field tag is correct. Have you sorted this out? Please confirm.

    With regards to the missing fields, your form contains a few fields, the best way I could offer to resolve that is to re-create the form notification settings. First, you need to delete the existing settings by following this guide: How to Delete Notifications. Second, re-create the form notification by following this guide: Setting Up Email Notifications.

    Let us know if you need further assistance.

    Thanks

    Hi there,

    That is indeed the correct form,

    I have also deleted the notification and created a new one, but the problem still remains.

    Hi,

    I can see the Notification 2 e-mail you created like this

    The form fields are on the right side, for any missing fields in the e-mail notification, you can add them using these fields.

    I can also see that the e-mail subject fields on it. Are you saying that the actual e-mail notification being sent is not similar to what is currently presented in the notification e-mail template?

    You can also try using the Test E-mail button to have a preview e-mail sent to you. This way you can check which fields are not being included in the actual notification message.

    Please inform us if issue persist.

    Thanks.

    Hi there

    the above is exactly what i've done. i've done a test email and that works, but as soon as i recieve a submission, the information is missing again within the notification. even though they have provided those details.

    The email Subject is missing {risktating40} even though i have put it in like you can see above. there also fields with the body missing also such as {ccglocality}

    any advice?

    @memaxts

    The correct tag for your form's "Risk Rating Calculator" field is {riskRating112} not {riskRating40}. Since the tag is incorrect, the email generated will not contain the field information you need.

    Please bear in mind that when you change field titles, the tag also changes. Since the email template has already been modified, it can no longer update the field tags.

    To double check whether the tag is correct, simply click on the corresponding field on the right menu. It will always generate the updated and accurate tag.

    Hope this answer helps.

    Hi,

    Thank you for providing us more details.

    I was able to find the field you were trying to use on the subject line. It was the "Risk Rating" field in form's Section Four - Risk Assesment.

    I am not sure yet where the problem lies why it would be empty in the actual notification e-mail.

    BUT the thing we are sure here is, the e-mail template is already customize when the field was used in the subject line.

    May I suggest you Add a New Notification E-mail and do not customize it.

    Just add again the {riskrating40} field into the subject line by clicking on "Risk Rating" field on the fields on the right panel. Do not type the field name manually.. you have to click on it to add it.

    Then test submit the form again. Check the e-mail message if the subject line includes the info.

    Please update us of the results.

    Thanks.
