This has been sorted out already. Your account was already re-activated after finding no violations of our terms. Your form should now be working fine.
About your last question concerning the suspension, when you're on a paid account, this will never happen. Paid accounts are manually checked by us unlike for free/guest users, our automated phishing filter suspends the account right way if there are detected forms that are positive of phishing attempt. I would recommend you to sign-up to one of our paid plan to avoid any inconveniences and interruption of the service.
