Our forms are mobile ready. But, we recommend our default form formatting to keep its responsiveness. Using a template with custom formatting, such as the background image would break the mobile functionality, so we do not advise that.
Did you check your form using your mobile device? Did you see any issue? Let us know so we can check further. If anything goes wrong, we can help you tweak the form to make it more mobile responsive - by adding custom mobile CSS queries.

We can go ahead and use the default formatting rather than the template with cusotm formatting. Howver, the selection fields are still a little too small and hard to select on some mobile devices. We'll try the customer CSS to adjust.
