lost access code

    for some reason, your form to retrieve my code is resetting itself.  Can you help?

    Hi Louise,

    We can retrieve the embed code for you. You have two guest accounts on file, so may we ask which form you're referring to? Kindly provide the form's direct URL. 

    You may also try to reset the web browser that you're using right now, or use another web browser altogether and try to retrieve your form code again.
