This issue might have been caused by conflicts with your jQuery Fancybox scripts and the scripts responsible for displaying the form, since you're using Embed <> scripts.

    Try using iframe codes instead.

To get your form's iframe codes:

    1.  Go to Setup & Embed tab
    2.  Click Embed Form
    3.  Click iFrame

    4.  Copy the codes provided in the succeeding screen

Let me know how this solution works for you.

    Hi mkhouzam

A search for a solution to your problem led me to this page. I don't know how good your JavaScript coding skills are but according to the link, it appears that this part of your source code needs to be edited in some manner. Alternatively, perhaps using Fancybox version 1.3.1 might also do the trick.

    Please keep us posted.
