My form has been working great, embeded onto a website and all.
Today, after I made a change to a text box of HTML text, I began having problems.
When I preview the form, it starts to load a disappears. The same thing happens on the website where it is embedded.
I attempted to disable and enable to form. Tried a different browser. Completely removed the text that I changed. Nothing seems to fix the issue.Page URL:

Update: As I trouble shoot this form, to no avaial, I am begining to recreate the form. After each section I save and preview. It is here that I notice that the "page break" element seems to be causing the above issue. As I insert the break, save, and preview the form it no longer displays (only a blank pop-up display window).

I checked your form that have the page break field issue http://www.jotform.us/form/33596241662155
And I found out that the scripts included in the html field is causing the issue on the form.
When I removed this fields in the form, the form was able to display properly.

I cloned your form and tested it. Here is a working version https://secure.jotform.me/form/33597034268461? after I have removed the mentioned fields.
I am not able to figured out yet which script is causing it.
BUT I would suggest you try amending the HTML field that combines the image and the texts. Try not to combine them and use image field and TEXT field separately instead. Similar to the same combination you did on the very 1st page.
See if this makes the difference.
