Hello Jotform Support,
I'm a subcontractor for one of your clients- Inspired Solutions (Denise Mayhan). We have quite a few forms as embeds for national convention registrations and have noticed that the form IDs seem to change from year to year without intervention from us. Of course when this occurs, we get a flood of calls from people who are unable to register.
Whould it be possible for Jotform to send us an email in advance of the ID changes so that we may prevent the disruptions? The following is an example of section of code where I am finding a difference between the form on your site and the embed:
Form Embed in Web Page (1 year ago, located at http://www.nbcusaregistration.com/laymen2013_old.htm):
<input type="hidden" id="simple_spc" name="simple_spc" value="20081922803" />
document.getElementById("si" + "mple" + "_spc").value = "20081922803-20081922803";
Same Form at Jotform Today (located at http://www.nbcusaregistration.com/laymen2013.htm):

document.getElementById("si" + "mple" + "_spc").value = "22358824248964-22358824248964";

After some search using the form IDs I found that those are actually two different forms.
It is possible that a new form was embeded to the webpage with a similar HTML/HTM file name. As you can see the names of the form are different, so, that means they are different forms and could not be a new one that was created automatically.
Please make sure that you embeded the correct form, but if still find issues/questions about this, do not hesistate to contact us.
