I own a few restaurants and was looking at embedding an customer evaluation form on my website. When I create the form on jotform, does it allow me to embedd it to my website. Also is that option for all pricing?
I also wanted to know if I created an evaluation form, if there was an option to change what email address the submitted form was sent to.

First, I would suggest creating an account with Jotform first so you can managed your forms easily with a sufficient free limits, respectively. Next, clone that form to your account by clicking "Use Template" button. http://jotform.ca/form-templates/restaurant-feedback-form. It should create a brand new form with the same template on your account.
