Inserting a background image can be achieved by injecting CSS Code in to your form.
Example Form: http://form.jotform.co/form/33606931590860
Ive injected the following CSS code to make the image appear on the background of the form:
.form-all
background:url('https://cms.jotform.com/uploads/image_upload/image_upload/global/22709_2013-bmw-m6-coupe-34_1600x0w [640x480].jpg')
no-repeat;
}This are the steps to access the Inject Custom CSS field:
On the Form Editor, go to, Preferences - Form Styles - Inject Custom CSS
If you need further assistance, do let us know. Thank you.
