My form has a few YouTube videos embedded from the YouTube widget that are not appearing on Chrome browser. In their place is blank space. They seem however to work fine on Safari.

I checked your form using chrome browser, but I was not able to reproduce the issue from my side, the youtube videos are shown with no problems:
So, in order to resolve this issue please clear your browser caches, that may help. Here is how to clear your browser caches: http://www.wikihow.com/Clear-Your-Browser's-Cache

Hmmm. Seems to be working for me now too. Maybe they take a little while to "kick in."
Thanks for your help!
