    The page is only showing page background, no form elements.

    After testing your form in an iPhone 5 emulator, I found that the form is not showing, however, I can still enter answers in the fields of the form:

    The standalone version of the form works fine too, even when it is a bit cut off:

    So, it is possble that some other element is overriding your form but it still allow to type in the fields somehow. So, in order to resolve this issue, please make sure to inject this code to your form:

    @media only screen and (max-device-width: 550px){

    .form-all

    {

    z-index:2 !important;

    }

    }

    Here is how to inject CSS codes to forms: How to inject Custom CSS codes.

    Please inform us if you need further assistance.

    Thanks

    Hi there,
    Thanks for the reply.
    I have added the CSS, but there is still a problem with iOS.
    The form does not show on page load. Interestingly, I just noticed that the form renders once I touch an area of the page where a form field sits.
    As well as this we also have a width rendering issue on iOS.
    Please advise.
    Regards,
    Gareth

    @Gareth

    Have you tried to embed your form using iframe embed code? This might fixed the form not loading issue. Please give it a try and let us know here if the issue persists. Thanks!
