I see the different account include different amounts of submission. How are submissions counted? When I have people submit a form can I export that information as a cvs file so that I can import it into my regular data base? I see there are many apps listed most of which I am not famliar with. I would like to give my clients a tablet with a jotform on it to fill out and the information be stored for me. What kind of app would I use for this? Also I have many more questions that would be great to ask over the phone. Do you offer phone support or just email?

When someone submits to your form, that will be counted as your submission. The limit is the total from all forms you have. Check our pricing page: http://www.jotform.com/pricing
Yes, exporting CSV is possible. Check this guide: How to create a CSV Report?
You can select a few from our Apps Section: http://apps.jotform.com/category/data_management. Generally, what is your preferred method of storing data? MySQL Database? If yes, you can export your form submissions as MySQL Format: http://apps.jotform.com/app/mysql_export

I think I read that the submissions are monthly totals. So you get 1000 submissions each month. Is that correct?

Yes, that is correct. And submission counter will reset every first day of each month.
