Yes, I was trying to create a form which would fill in the CMS pdf. The answers would have to go exactly where they are located on the PDF in the final layout but the actual form is not required, just the data.
Whenever I try to upload the pdf it just spins and never loads

I tried to work on this but it is just too difficult with this editor. The data has to be in the exact spot as on the form and it would require changing question types and assigning (x's) over so many fields that it would take forever.
Since this is the form that all US providers use to submit claims, is there anyone at Jotform that works with healthcare that would like to work on this with me?

Unfortunately, due to HIPAA restrictions we cannot login to your account and edit your form.

As long as you have fillable fields, the Smart PDF form builder would map the fields accordingly.

Please note that if you use a widget, then you must manually map those fields to your fillable PDF.
