I tested again with IE 11 and now I am receiving 4 identical confirmation emails and 4 identical notification emails on each form I submit - outpatient ultrasound, analog radiograph and digital radiograph. This is not the case with Chrome or Firefox - where I only receive 1 email of each type. I have updated to the latest version of IE 11 and using it to test (11.0.9600.16428).

I don't know whether it matters - I am using my gmail address for my confirmation email (doctor's reply to email in referring veterinarian section) and my work email (zimbra) as the auto route for the notification email. Again, this is not happening when I submit the form through firefox or chrome.
Can you help?

update: I receive (only) 3 notification and 3 confirmation emails when I submit the analog radiograph form in IE 11. but 4 duplicates for outpatient ultrasound and digital radiograph.

Just to clarify: This only happens when you fill out and submit the form using Internet Explorer, right?
I am currently running some tests using a copy of your form, making submissions on IE11 using Windows 7 to attempt to reproduce the issue.

Since the form is working fine in other browsers, it is not an issue in the form itself. It is an issue related to the IE 11 not being compatible with some script of the form and that is causing the multiple submissions. Did you already try testing a different simple form? That would help us to determine whether the problem is with every form or just with the ones that have conditional emails.
If the problem does not occurs with a simple form, that means that the script of the conditions are not compatible with IE 11 and that is causing the issue. So, in that case it would not be recommended to use IE 11.
So, please try using a different form and inform us if the same issue happens, and in the mean time we will keep testing your forms try in to find some good results.
