Hello,
There are two ways to add a QR code to your form.
1.By using our Qr Code Widget in Form Build;
- Go to Form Builder and click Add Form Elements.
- Go to Widgets and search for QR Codes.
- Then, you can enter your Message to code and you can change the setting from the properties. Here is a screencast for you!
2.If you already have a QR code, by using the Image element in Basic Form Builder.
- Go to Image in Basic Form Elements.
- Form choose a file, upload your QR code image, and save!

To clarify, do you want to create a QR code of your form so that when they view the code, it will open with the form?
You may share the form using QR code.
