For verification purposes, may we ask which email address you used to pay for the upgraded JotForm? Right now, your registered email address is email@example.com.
Can you confirm that you used the same when you paid for the subscription? you should use the same email address for all payments to avoid any issues.

It appears that you have used different email addresses when you paid twice for a Premium account. With your permission, we can refund the amount you paid erroneously.
Your current account - bmbb registered under firstname.lastname@example.org - reached its maximum submission limit since you now have 1106 submissions as of this writing. You can either upgrade to a Professional account or wait for the next cutoff date to reset the monthly submissions to zero.
