I've been away from JotForm for some time and I can't find any references to how to set-up a photo in my form. This sounds like a simple problem but it seems to be undocumented in the FAQ or manual. Search word "upload" doesn't even deserve a response!
Do I need a directory set up to receive the file(s)? Can I embed the image into the email? etc?

Are referring on adding your own image/logo in your form? If yes, to add your own image/logo in your form, just simply use Image field which can be found under Form Tools.
You might like to take a look at this demo form that I've created which I was able to add my own image/logo without any problems.

In addition to my response, please try also clearing your browser cache and cookies if you haven't tried it, then upload again your image/logo using Image tool.
