    When I try to open my form on my website it says "404 Not found"

    I can't pull up the HOME Choice Payment request. Jot form says 404 Not found.

Can you please give us its form link or the link you are accessing? as I can not pull up any information from your email address that you included when making this inquiry.

    Here is the website. If you choose HOME Choice-payment Request, you get a 404 Not found error.

    Thanks for the explanation.

    Your form is giving such error because of the ".com" added at the end of the URL of the form, see this video: 


    So, make sure to remove the ".com" from the URL when linking to the form using the tags for hyper links:
