Phishing form hosted on your service violating ToS

    I am a representative of Roblox, an online gaming company. The form at

    appears to target our users in a phishing attempt. In addition, it is using our copyrighted logo for Roblox, and misrepresenting the owner as a Roblox employee. Our logo can be viewed at

    Roblox has no affiliation with this form. We would like to request under the DMCA that it be removed as quickly as possible.

    I, under penalty of perjury, swear that the information contained in this notification is accurate and written as an official representative of Roblox.

    Thank you for bringing this to our attention. For some reason, the said form managed to evade our Phishing Detector. Jotform has a zero tolerance policy when it comes to illegal online activities. As such, I have suspended the responsible account and deleted all entries submitted on that said form.

    We sincerely apologize for any inconvenience caused by this issue.
