    Hi there,

we are happy to help you with this.

A workaround to achieve this would be to add all the lines to the multiple-choice widget as separate options and then hide the checkboxes for the ones you want as information only.

Please check out this demo form that I created for you: https://form.jotform.com/201333763858056

If you like it you can clone it, or just examine the build and CSS and apply it to your own form.

I hope this helps, please let us know if you need assistance with anything else.

    Thanks

    Greetings.

If you wish to add additional text descriptions, you can add a Paragraph field, and shink them by right-clicking on them to set them next to eachother.

You can check the following example form:
    https://form.jotform.com/211514358073047

Can you please let us know the URL of your form so we could check it and let you know how to reduce the spacing between the header and question?

    Thank you for your cooperation.
