I was really, really, upset after spending 3 hours designing a form, previewing it, iFrame embedding it in my website and seeing it work fine in my browser....then it DISAPPEARED!!! It was replaced by a COMPETITION TEMPLATE which had been my starting point.
I no longer trust JotForm.
I have no choice but to start from scratch.

I'm sorry for the trouble caused to you.
I would like to assure you that JotForm does not delete any form. Even if some form is suspected of being used for phishing, we simply disable the form and suspend the account but never delete any form without user's consent.
You must have logged in to your account from more than one browser window and this particular form must have been loaded in the editor. Please be informed that we have auto save feature in form editor which will always overwrite the other version of form.
If you are working on a complex form, you should always clone it in regular interval. Cloned version will always work as a backup for you. This guide should help you in form cloning: http://www.jotform.com/help/27-How-to-clone-an-existing-form-from-your-account
