    Hello ankuraffinity,

    Upon checking your web page, I am unable to replicate the problem you are having. On click of the button "CLick Here to Apply", your form loads fine. Please check the screenshot below:

    Have you resolved your problem already? I did check with multiple browser and your form loads just fine. I also sent you few test submissions without any problem.

    Do check and get back to us if the issue persists.

    Thank you!

    Hi Ankuraffinity,

    Looking at your provided link, it seems either your form is no longer embedded to your website or it has been deleted as you are assuming. Well, luckily both conditions are recoverable. Please follow below steps;

    1) Login to your jotform account and go to My Forms.

    2) If you do not see your respective form in My Forms page then click on Trash on the left panel menu. If the form has been mistakenly deleted, it will appear in Trash.

    3) Click on your form check box and click Restore button as shown in below image.

    4) This will restore your form and it will re-appear in My Forms page. Now, edit your form and embed it again from the Embed option on the top of the form.

    I hope this will resolve your problem.

    Regards,

    Sam

    Hi, I just checked it.I was trying to find this form in my forms section of the footer But this section shows only 5 forms at once so I thought it is missed.But It was there on this link http://www.jotform.com/myforms/

    I have another problem.If you can see in my account.I have set a condition that if someone will select "Other" in Location option then it will show the field "other location".

    Bit it is showing automatically.
