I’ve asked this question before on December 30, but the problem seems to have continued.
My form has a few YouTube videos embedded from the YouTube widget that are not appearing on Chrome browser. I have checked using multiple computers and operating systems. In their place is blank space. The videos appear fine using another browser like Safari.

Upon viewing your form I was able to reproduce the issue, however, this is due to the fact that you are using the your form in Secure mode (HTTPS://SECURE). Our widgets do not support secure protocol currently, so your videos will not display if you use your form in secure mode.
So, I have forwarded this issue to our dev team so that they can find a prompt solution to this bug. We will inform you via this thread once the issue is resolved.
If it is not mandatory for you to use the secure mode of your form you can change your form URL to this one http://www.jotform.com/form/33633992009862
