Hello,
We are having an issue with a popup window (explaining allergies) that is preventing patients from being able to complete their forms online. Can you help remove the pop up window?

Could you kindly let us know the specific fields you are referring to? In addition, does it happen on the direct form or does it happened at the embedded form? If you have embedded the form somewhere else, kindly share with us the website URL.

So, the pop up window is not happening if you use a phone to fill out form, just the desktop. But it is a window explaining allergies, but it blocks patients from filling our questions 9 and 10. I did not create this form, but I have copied and duplicated it several times without issue, until today. I am not sure where the pop up window explaining allergies is coming from. Are you able to remove it??? Or can you advise me? I can’t figure out where it is coming from. But if you try to fill out our form, you will clearly see that the window is preventing you from answering questions 9&10 and therefore cannot submit the form.

On the builder page, here's how you can remove the hover text that appears on the Input Table of the form you linked.
Just erase the entire text in the box and test your form again after.
