Is there an alternative to multi-select that can work for my form?

I am using a multi select grid and need users to be able to select the same options. Is this possible? Or is there a better tool?

After taking a look at your form, it seems like the Configurable List widget is going to be your best bet. Here's a demo form of what that can look like.

Users are able to add lines as needed and the drop-downs and text areas are full configurable. Details on how to configure the fields in the widget is available here. The settings that I used in the widget are as follows.

Name of Activity : text

Freshman : dropdown : Yes, No : Please Select

Sophomore : dropdown : Yes, No : Please Select

Junior : dropdown : Yes, No : Please Select

Senior : dropdown : Yes, No : Please Select

You add this under the widget's settings.
