The submit button stopped showing

I'm unaware of making any changes that would've caused this jotform to no longer show the submit button but for some reason, it isn't showing. I've scoured through the conditions and can't figure out why it doesn't show. Please take a look at it and tell me what's wrong. Thank you

Please review your conditions. There are some conditions that are hiding the page breaks and this not letting the users move forward.

You may write conditions to show hide the fields avoiding the page breaks.
