I am able to edit and save my form, but no one can fill it in. Last submission I was able to receive was last week. Please help!

I'm sorry for the trouble caused to you.
Upon checking your form, I found it to be disabled. This is the reason why no one can fill and submit your form.
Here are the steps to enable your form:
1. Click on "My Forms" from the top menu.
2. Select the form which you want to enable.
3. Click on "More"
4. From the drop down, click on the "Enable" option.
